Type of event: accidental exposure to radioisotope
During operations involving filling of gaseous tritium light sources, a broken inlet manifold resulted in the escape of 11-15 TBq of tritium, causing exposures to two individuals; whole body doses were 62 rad and 64 rad.
Consequences: 2 injuries.
